  8. 08Extraction & closure

Four ports, one non-negotiable: the critical view of safety before anything is clipped.

Step by step

  1. 01

    Time-out & positioning

Supine, reverse Trendelenburg, left tilt. Confirm antibiotics and imaging.

  2. 02

    Port placement

Open Hasson at umbilicus; epigastric 10 mm and two right subcostal 5 mm ports under vision.

  3. 03

    Fundal retraction

Fundus over the liver toward the right shoulder; Hartmann's pouch lateral and inferior to open the triangle.

  4. 04

    Hepatocystic triangle dissection

Peritoneum opened anterior and posterior; stay above Rouvière's sulcus.

  5. 05

    Critical view of safety

Two, and only two, structures entering the gallbladder; lower third of the cystic plate cleared.

  6. 06

    Clip & divide

The cystic duct and artery are clipped and divided only once the critical view of safety is confirmed.

  7. 07

    Dissection from liver bed

Energy in the areolar plane; keep the cystic plate intact.

  8. 08

    Extraction & closure

Retrieval bag via umbilicus; close fascia at 10 mm sites.
